We know helping your pet at the end of life is probably one of the hardest decisions you have ever had to make. We are here to help. Our service is completely designed with your pet’s comfort in mind. Allowing them to be at home surrounded by the ones they love, eating treats and sparing you, the pet parent, that difficult car ride taking your pet to a place they fear. We also know it can be difficult to talk during this time, so we have set up this form to allow you to begin the conversation by answering some questions to get us started.

How this works…please complete this questionnaire for home euthanasia, cremation or an enrollment visit for palliative care (note that overnight coverage is no longer offered as part of our true pet hospice (Pet Parent Peace of Mind Program). Once you submit the completed questionnaire, an intake coordinator will call you shortly to answer any other questions you may have and confirm an appointment time if you are ready.
